Good dental health is an excellent investment in an individual’s medical and psychological care.
Please remember you are ultimately responsible for all fees charged by this office regardless of your insurance coverage. Payment arrangements should be made prior to treatment. The Patient’s estimated portion or copayment for treatment is due at the time of service.
Major Service-Two Payment Option: For crowns, bridge work, dentures or any other treatment that will require two or more appointments to complete, we accept one-half of your copayment at the first appointment and the balance at the delivery appointment.
Patient Financing: Upon approval, we offer our patients an interest free term loan with no down payment, no annual fee or prepayment through the Care Credit program. Please ask us for an application.
MISSED APPOINTMENTS: The dental appointment represents a shared responsibility for both the Doctor and Patient. In order to provide quality dental care at an affordable cost, scheduled appointments must be kept. We strongly encourage all patients to make every effort to keep their appointments. Should you need to change an appointment, we ask that you give the office 24 hours notice. There are often agreements between groups and insurance companies that call for a copayment for broken appointments.
